WORK EXPERIENCE

In order for a student to participate in School Sponsored Work Experience, they must complete and return the three documents below. In addition, students must successful complete Learn to Earn prior to enrolling in Work Experience OR demonstrate the skills required for other forms of evaluation. 

DOCUMENT #1--EMPLOYMENT AGREEMENT

  1. Complete all of the information on the top of the form.

  2. Have your employer read the employer expectations, then, have your employer sign and date the form at the bottom.

  3. Have your parent/guardian read the parent/guardian expectations, then, have your parent/guardian sign and date the form at the bottom.

  4. You read the student employee expectations. Then sign and date the form at the bottom.

  5. Return the form to Mr. Christenson. He will take care of getting the school signatures.

DOCUMENT #2--WORK EXPERIENCE PROGRAM RULES

  1. Sit down with your parents and read the School Sponsored Work Experience program rules. Then your parent/guardian and you sign the form on the back. By signing, you are agreeing to abide by program rules. (Only one parent/guardian signature is required.)

  2. Return the form to Mr. Christenson. He will take care of getting the school signatures.

DOCUMENT #3--TRANSPORTATION AGREEMENT

  1. If you plan on driving a vehicle to and from work each day, this form needs you be completed and returned.

  2. Parent/guardian name or names go on line three.

  3. Student’s name goes on line four.

  4. Student’s name also goes on the “Parent’s Designee” line.

  5. Parent/guardian must sign at the bottom.  IMPORTANT--this form must be notarized. Mrs. Deiss in the high school office can notarize documents.

IMPORTANT THINGS TO REMEMBER

  1. All forms must be returned to Mr. Christenson before you will be released from school to participate in the program.

  2. The school reserves the right to approve the place of employment if the place of employment does not meet standards as designated by the school.

  3. If there are days a student is not scheduled to work, the student must be in school all day.

  4. Students participating in School Sponsored Work Experience must be passing all classes.

  5. When driving during the school day, students are prohibited from providing rides to work for fellow students unless the school has written permission from the parents/guardians of both the driver and the passenger.

  6. Students must be in attendance all day in order to be released to participate in the program.
